entryId = SubModule enum name (parent row) or PermissionEnum name (OTROS child row); actionId = ALL or a TipoPermiso. Identifiers are whitelisted against the gesmo-permissions enums. Anti-escalation: granting requires the caller to hold the permission; revoking is unrestricted.
